I give my permission for my son/daughter to participate in FSCC Fit Kids Camp taking place at Fort Scott Community College knowing that it will contain vigorous physical activity that could lead to injury. I hereby authorize the Directors/Leaders/Coaches/Staff of the Fit Kids Camp taking place at Fort Scott Community College to act for me according to their best judgement in any emergency requiring medical attention. I know of no mental or physical problems that may affect my son or daughter's ability to safely participate in the Fit Kids Camp taking place at Fort Scott Community College. I hereby waive and release the Directors/Leaders/Coaches/Staff of the Fit Kids Camp and Fort Scott Community College from any liability, illness or injuries while participating in the Fit Kids Camp taking place at Fort Scott Community College.
